Survey About School in Fall

Dear Crescent family,

Thank you for all of the love and support you give us everyday as we grow our Crescent cubs together. We need your feedback on this survey to look for options to help and support all of our Crescent students in the fall. The survey closes June 27th please take a few minutes and complete it so we get as many responses as possible.

 Canyons District is making important back-to-school plans to address COVID-19-related health, wellness, and safety issues. We need your input and perspective. Please follow the link below to respond to a short survey, which will take approximately 5-7 minutes to complete. Because each child’s needs may be different, parents will receive one survey per student enrolled in a Canyons school. You can select your language preference at the top of the survey.
